Agreement to Pay

Agreement to Pay forms are required for all applications (including fixed fee cases) Telecom Carriers may hire Agents to do permit processing on their behalf. However, the Carrier is responsible for payment of costs and fees Agents may submit payments on behalf of the Carriers; however refunds and reimbursements will be sent to the Carrier Should the Carrier change during the permit processing, a new Agreement to Pay form must be submitted Agreement to Pay – This form should be used if the Carrier is being billed directly Agreement to Pay-On Carrier Behalf – This form should be used if an agent/company is paying on the Carrier’s behalf
